Ameritas dental plans and how it explains PPOs and plan terms

Ameritas defines a PPO as a group of dentists who agreed to charge less than their regular rates, says members can see any licensed dentist with the most savings in network and says you can ask the insurer to invite your dentist to join. It also sells individual dental plans at three levels.

Ameritas explains dental insurance in plain terms on its own pages, and those explanations are worth reading whichever carrier you have. It defines a PPO as a group of dentists and specialists who agreed to join the insurer's network and charge less than their regular rates, and it says members can see any licensed dentist, with the most savings in network. Ameritas says that if your dentist is not in the network, you can ask the insurer to invite them to join. It sells individual dental plans to people without employer coverage, with three plan levels, including one with a child orthodontic benefit. Before treatment, settle whether your dentist is in-network, whether it is worth asking Ameritas to invite an out-of-network dentist to join, and which of the three plan levels actually includes the orthodontic benefit you need; plan-level dollar amounts change and are left out here for that reason.

What a PPO is

Ameritas's four points about a dental PPO

FactorDetail
A large networkA group of dentists and specialists who agreed to join the insurer's network and charge less than their regular rates. Members can see any licensed dentist, including those outside the network, though they get the most savings in network.
You spend lessContracted dentists agree to discounted rates. Members pay a portion of the discounted price and the plan covers the rest, in the form of a deductible or a percentage that differs by procedure.
Claims are managedNetwork dentists file claims for members, usually electronically. Ameritas adds that out-of-network dentists sometimes file as a courtesy.
Know costs in advanceAmeritas says to ask the dentist what amount the plan covers before scheduling, and that dentists can submit a pretreatment estimate to the insurer.

Ameritas also says PPO providers may offer discounted rates on non-covered services as approved by state law. Ask the office whether that applies.

If your dentist is not in the network

A step most people do not know exists

Ameritas points out that many people have a preferred dentist and do not want to switch. Its suggestion: if your dentist is not in the network, you can request that the insurance company invite the provider to join, and see if the provider accepts. That is not a guarantee, and a dentist may decline. It is a real option, though, and it is free to ask. You can check whether a dentist is in-network by searching the insurance company's online directory, and you should do that before you enroll.

Ask the dental office whether it would consider joining a network, and what the fee difference would mean for its patients.

Individual plans

What Ameritas says about buying on your own

FactorDetail
Who they are forAmeritas describes individual dental and vision plans for people without employer benefits, including dependents aging off a parent's plan, self-employed people, seniors and part-time or contract workers.
Three dental plansA basic plan for those with good oral health, a mid-range plan with a child orthodontic benefit, and a fuller plan with a generous maximum and hearing benefits.
RatesAmeritas says rates are based on location instead of age.
EnrollmentAmeritas describes simple online enrollment, sold partly through agents.

Plan terms

Ameritas's glossary of terms that decide your cost

FactorDetail
Annual maximumThe total the plan pays each year for each covered person. Once you reach it, additional costs are out of pocket until the next year.
Lifetime maximumA one-time spending limit, typically used for orthodontic treatment. Once reached, the plan pays no more of those services for the member's lifetime.
DeductibleThe amount you pay upfront before the plan starts covering treatment. Only eligible costs count toward it, and it can be per year or per lifetime.
CoinsuranceAfter the deductible, the plan usually covers a percentage of each service and you pay the remainder.
Calendar year or benefit yearSome plans follow the calendar year and others start and end at different times. Knowing which helps you time treatment.
Waiting periodSome treatments may have a waiting period before coverage begins, often for costly services such as crowns.

Insurance or discount plan

How Ameritas describes the choice

Ameritas describes discount dental as not technically insurance. Members usually pay a monthly or yearly membership fee and receive discounts from a specific network of providers, and Ameritas says a discount plan may be fine in the short term for healthy people who do not need much dental care. It describes PPO plans as having an annual maximum, DHMOs as paying dentists monthly to provide covered services at no or low cost with members limited to the approved network, and indemnity plans as letting members see any dentist and pay what the plan does not cover at the dentist's usual fee.

It also notes that direct-reimbursement, point-of-service, limited-network and scheduled-benefit plans exist.

Ameritas is an insurer, and this is its view. Compare the terms of each product yourself.

Questions people ask

Common questions

01

What is a dental PPO according to Ameritas?

A group of dentists and specialists who agreed to join the insurer's network and offer services at lower than regular rates. Members can see any licensed dentist, including out of network, and save the most in network. Ameritas says network dentists file claims for you and that members pay a portion of the discounted price.

02

What if my dentist is not in the Ameritas network?

Ameritas says you can request that the insurance company invite the provider to join and see if the provider accepts. Nothing is guaranteed, and a dentist may decline. You can also see a dentist outside the network, where you get less savings. Search the online directory before you enroll to see who is in.

03

What is a lifetime maximum?

Ameritas describes it as a one-time spending limit, typically used for orthodontic treatment. When the plan reaches it, it will not cover more of those services for the member's lifetime. It is separate from the annual maximum, which resets each year. Ask your plan whether orthodontics has its own lifetime maximum.

04

What is the difference between a calendar year and a benefit year?

Ameritas says some plans operate on a standard calendar year, while others start and end at different times. Knowing which applies tells you when your annual maximum and deductible reset, which matters when you are planning treatment near the end of a year. Ask your plan for the exact dates.

05

Does Ameritas sell individual dental plans?

Yes. Ameritas says its individual dental and vision plans are for people without benefits through an employer, and lists three dental plans from basic to more comprehensive, with rates based on location and not age. The page is written for agents, so check the current plan documents for benefits, waiting periods and limits.

06

How do I know what a dental procedure will cost me?

Ask the dentist what amount the plan covers before scheduling. Ameritas says dentists can submit a pretreatment estimate to the insurance company, and that it offers a dental cost estimator tool. Compare the estimate with the office's quote, and remember that final payment can differ once limits apply.

07

Is a discount dental plan a good alternative?

It depends on your needs. Ameritas says discount dental is not technically insurance and may be fine in the short term for healthy people who do not need much care. You pay the discounted fee yourself, so large treatment costs more than it would with insurance. See our dental discount plans guide.

08

Do PPO dentists offer discounts on services my plan does not cover?

Sometimes. Ameritas says PPO providers may offer discounted rates on non-covered dental services as approved by state law. Ask the office whether that applies to the service you want, and get the discounted fee in writing before you agree to treatment.

09

What percentage does a PPO usually cover?

Ameritas says preventive checkups and cleanings are typically covered in full, basic procedures at a middle level and major procedures at the lowest of the three. The actual percentages come from your plan. Read the schedule of benefits, and use the insurance benefit estimator to see how the numbers combine.

10

Can I change dental plans every year?

Often, yes. Ameritas notes that dental plans have an annual renewal, so people can change plans or carriers to fit changing needs. Check when your plan renews, how a new plan's waiting periods would apply and whether treatment in progress would be affected before you switch.

11

What kinds of dental plans does Ameritas describe?

Ameritas describes PPO plans, DHMOs, discount dental, indemnity plans, direct-reimbursement plans, point-of-service plans, limited-network plans and scheduled-benefit plans. Each has different rules about networks and maximums. Ask which one you are being offered and read the plan documents for the limits.

12

Does Ameritas offer a dental cost estimator?

Ameritas's page says its plans come with a dental cost estimator tool. Use it to prepare, then ask the dental office for a written estimate and ask Ameritas for a pretreatment determination on large work, since the final payment can differ once limits apply.
